GP and health professional referrals are welcome.

Integrated care between psychologists, medical practitioners and allied health clinicians improves outcomes for patients navigating health challenges, adjustment difficulties and psychological distress. Case discussions and case conferencing are welcomed to support coordinated care.

Located in Upper Mt Gravatt, our practice provides a convenient referral option for GPs and health professionals supporting patients across Brisbane's Southside.

Referral pathways

How to refer

GPs and health professionals may send referral documentation including Mental Health Care Plans (MHCPs), GP Chronic Disease Management Plans (GPCDMPs), or referral letters via the practice email or usual secure referral pathway.

Mental Health Care Plan (MHCP)

Standard referral pathway for psychological services with Medicare rebate eligibility.

GP Chronic Disease Management Plan (GPCDMP)

May be appropriate for patients with chronic or complex health conditions.

Insurer-Funded Services

Support may be available through approved workplace, motor vehicle, and other compensable claims.

GPs may provide referral letters through their usual referral pathway.

About the practice

Working together

Health Nexus Psychology provides evidence-informed psychological support for adults experiencing health challenges, adjustment difficulties, chronic health conditions, workplace concerns and psychological distress.

We work alongside existing medical care to support patients in building coping strategies, adjusting to changes in health status, improving health-related behaviours, and maintaining work participation where appropriate.

The practice is led by a Registered Psychologist and Health Psychology Registrar undertaking advanced training toward endorsement in Health Psychology under the Psychology Board of Australia's Registrar Program.
